On 2 July 1994, USAir 1016, DC9-31 collided with trees and a
private residence while attempting a missed-approach for the ILS
runway 18R at the Charlotte/Douglas International Airport at 18:42:36
EDT.

Above vector diagram shows the relationship of acceleration on
the pilots at the time when the captain said ÒDown, push
it down.Ó The captainÕs otolith was sensing
that the body was pitched up 15 + 10 = 25 deg, instead of actual
aircraft pitch attitude of 15 deg up. Because the captain
was flying visual (without visual cues), this lead to commanding
pitching down to the copilot, and decreased the aircraft pitch
attitude from the desired 15 deg up for microburst evasion.
This is an example of somatogravic illusion, which took the form
of over rotation illusion, type I. Somatogravic illusion
occurs from misinterpretation of linear acceleration stimuli from
otolith organs.
In this particular case, pitch sense model at the Brooks produced
a very flat pitch sensation for the pilots, which was calculated
from flight data recorder data.
